Abstract : To compare the efficacy of Manual Anal Dilatation versus Lateral Anal Sphincterotomy in the treatment of anal fissure. To compare postoperative pain between Manual Anal Dilatation and Lateral Anal Sphincterotomy. To compare the rates of incontinence after Manual Anal Dilatation and Lateral Anal Sphincterotomy. To compare the recurrence rate between Manual Anal Dilatation and Lateral Anal Sphincterotomy. Study was carryout in the Govt. General Hospital, Nizamabad with Patients with department with symptoms & signs of anal fissure. Taken sample size 58 with Hospital based Prospective Randomized Comparative study and study done between November 2020 to December 2021. Men to women ratio having anal fissure is 3:2. Complications are seen to be more with manual anal dilatation than lateral internal sphincterotomy. Recurrence was found to be more with MAD than open LAS. anal dilatation has more chance of incontinence when compared to LAS. However, controlled anal dilatation had less chance of incontinence when compared to LAS. Wound healing is better in LAS than MAD. Patients who underwent LAS had less postoperative pain when compared to MAD.
