Abstract : Upper eyelid blepharoplasty is one of the most common plastic surgical operations which is performed under local anesthesia. An ideal anesthetic agent in addition to analgesia and suitable duration of action should have fewer systemic and local side effects during and after surgery. The aim of this study is to compare the effects and side effects of lidocaine/adrenaline with ropivacaine in candidate patients for upper eyelid blepharoplasty. This research has been performed as a clinical trial. Forty candidate patients for upper eyelid blepharoplasty undergoing local anesthesia were randomly assigned into two 20-member groups. In the first group (intervention), ropivacaine and in the second group (control) lidocaine/adrenaline were used with the similar volume as local anesthetic agent. Then, the level of pain was captured through VAS, while the bleeding and edema were determined through clinical examination and recorded. The mean maximum concentration of the anesthetic agent required in both treatment and control groups was 4.7±0.4 and 4.8±0.4 cc respectively (P=0.455). In investigating the extent of analgesia, it was observed that across all study time intervals and in both intervention and control groups, the mean VAS of patients was less than three and no significant difference was observed between them (P=0.374). Concerning postoperative edema, in the intervention group, a clear reduction was observed in the edema around the eyes when compared to the control group (P=0.015). Ropivacaine as a local anesthetic agent in upper eyelid blepharoplasty causes diminished pain and incidence of postoperative edema. Further, it does not need adrenaline for controlling bleeding during surgery. Therefore, it can be used in high-risk patients for whom it is not possible to use adrenaline
