Abstract : In the words of T.S. Eliot,” Only those who will risk going too far can possibly find out how far one can go”. While fixed orthodontics is still the treatment of choice there are problems in relation to esthetics as well as in maintaining oral hygiene. Clear aligners have been designed keeping appearance and comfort in mind and at the same time can achieve esthetic results which are comparable to what we can achieve with fixed orthodontics. This case report depicts the use of aligners for treating a patient with Class I malocclusion. The patient had an orthognathic facial profile and visited the clinic mainly for esthetic concerns and wanted to correct her lower anterior crowding. Instead of fixed orthodontics the patient was asked to wear aligners and correct minor crowding present and then go in for veneers to minimize tooth reduction for the same. The patient was also looking to improve her soft tissue profile so initial estimation of her frontal soft tissue using the Modified Fitzpatrick Wrinkle Scale (MFWS) showed a score of 2.5. A total of 25 aligners were used for treating the patient and on completion her MFWS rating improved to 0.5 which was seen as a significant improvement on her earlier score of 2.5. There were also corresponding improvements in her soft tissue cephalometric values such as her Naso-labial angle and lip to E-plane values. Published literature has shown a strong correlation between various dental, skeletal changes and the soft tissue profile. Analysis of the case was done using the Modified Fitzpatrick Wrinkle Scale (MFWS) before and after the treatment to find out the improvement in soft tissue profile. Studies have shown the Modified Fitzpatrick Wrinkle scale to be a reliable clinically validated method to analyze the changes in skin folds before and after dermatological treatment.
