The Emerging of Extensively Drug-Resistant (XDR) Acinetobacter baumannii in Central Java Province – Indonesia
Abstract
: As one of the most antibiotic resistant bacteria, Acinetobacter baumannii (A.baumannii) becoming a serious threat to the global society, moreover due to the emerging of XDR A.baumannii. The study aimed to figure the A.baumannii resistance pattern and XDR A.baumannii’ prevalence, since there is only limited study published for this topic in Indonesia.We analyzed the antimicrobial agents’ usage for acquisition of A.baumannii in an observational descriptive study. This study conducted in one of the national referral hospitals, Central Java-Indonesia. We used data registered between January 2017 until December 2018, from microbiology laboratory which using VITEK® 2 as an antimicrobial susceptibility testing system. We presented the results using data classification and tabulation, which being analyzed from 699 samples that isolated from various specimens. The antibiotics susceptibility result of A.baumannii for group A primary antibiotics were: ampicillin-sulbactam 47.2% resistant; ceftazidime 65.5% resistant; ciprofloxacin 63.2% resistant; meropenem 47.7% resistant; gentamicin 60.4% resistant. While for group B primary antibiotics were: amikacin 17.9% resistant; piperacillin-tazobactam 61.7% resistant; cefepime 63.1% resistant; ceftriaxone 91% resistant; trimethoprim-sulfamethoxazole 18.5% resistant. Most A.baumannii were found in sputum (57.3%) and from pulmonology department (37.2 %). Among all specimens 90.6% were MDR and 9.4% were XDR A.baumannii. The antibiotics chosen for A.baumannii infections in group A were on the warning zone according to the hospital consortium, while in group B only amikacin and trimethoprim-sulfamethoxazole were still sensitive. The high percentage of MDR and the finding of XDR A.baumannii should raise awareness among all the healthcare providers in Indonesia.
