Abstract : Nasopharyngeal carcinoma (NPC) is commonly known to metastasized to bone, liver and lung. Nevertheless, distant metastasis of NPC to subglottis is extremely rare. We report a case of such disease in a 45 year-old lady with underlying NPC who presented with hoarseness, dyspnea and stridor after 6 years of treatment. Tracheostomy was done to relieve the distress and direct laryngoscopy revealed a subglottic mass with anterior extension to the neck. Histological and immunohistochemical analyses of subglottic specimen confirmed it is nasopharyngeal in origin
