Abstract : Non-compaction of left ventricle is an uncommon autosomal dominant disorder and Right ventricle non-compaction is still rarer. It is a rare cardiomyopathy that is believed to arise from an arrest in embryonic endomyocardial development. More recent studies suggest that it can be acquired later on in life sporadically. It may be accompanied by life-threatening complications, which are most commonly heart failure, arrhythmias, and thromboembolic events. We present a case of 40year old man who presented with dyspnea and palpitation since 15 years and was diagnosed after 2D Echocardiography and Cardiac MRI as Bi-ventricular non compaction with Atrial Septal Defect (ASD). To the best of our knowledge, this combination of ASD with biventricular non-compaction has not been reported in literature so far..

Abstract : Hepatitis has become a foremost health concerns worldwide. Majority of the infected population remain unaware of their disease status due to non-screening practices. The aim of present study was to evaluate the prevalence of undetected cases and practices regarding Hepatitis B & Hepatitis C among patients attending community pharmacies of southern Punjab, Pakistan. A cross sectional study was conducted for the duration of three months from January to March, 2017 using a structured, validated questionnaire. Rapid diagnostic kits were utilized for screening of patients from selected community pharmacies of two big cities of South Punjab, Pakistan. Descriptive and inferential statistics were applied for data analysis by using SPSS version 20.0. P value <0.05 was considered to be statistically significant. Among total 794 respondents, 565 (71.2%) were males and 229 (28.8%) were females. Prevalence of undetected HBV cases was 2.01% (n = 16) and for HCV was 5.41% (n = 43). Results showed poor practices of respondents with mean practice score of 2.54/6 (±1.581). Study emphasized the need of educational programs along with motivational campaigns to persuade people for regular screening of HBV & HCV that will prove beneficial in early diagnosis and management of the disease..

Abstract : Airway managements by direct laryngoscopy and endotracheal intubation during general anaesthesia are known to induce clinical changes in hemodynamic variables such as tachycardia, hypertension, and arrhythmias, which can cause myocardial ischemia. In order to prevent adverse cardiovascular responses, laryngeal mask airway can be used instead of laryngoscopy and tracheal intubation during airway management. The aim of our study is to compare the hemodynamic response with FASTRACH intubating laryngeal mask airway (ILMA) and Macintosh laryngoscopic intubation. Institute Ethics Committee Clearance was obtained before start of study. Study was carried out on 78 patients belonging to ASA grade I and II, aged between 18 to 65 years, including either gender. Patients were divided into Group M:Intubation using Macintosh laryngoscope and Group L:Intubation using FASTRACH LMA. After obtaining informed written consent from patients in their own understandable language, they were randomly assigned to one of the two groups based on WINPEPI APP. Mean heart rate when compared showed significant increase in group M whereas there was slight increase noted in group L. Systolic blood pressure and mean arterial blood pressure showed increase in both group after intubation. Diastolic blood pressure showed significant increase in group M whereas in group L significant decrease was observed. There were no ECG abnormalities noted in both groups. Based on our study we concluded that intubation using FASTRACH ILMA is associated with less cardiovascular response and hemodynamic changes when compared with Macintosh laryngoscope..

Abstract : Although weaning forms a crucial phase of an infant's life, there exists a data vacuum in the scientific literature regarding the awareness, perception, and onset of weaning among caregivers in the Kingdom of Bahrain. This study aimed to fill in these gaps and expand the information base of caregivers and clinicians by providing valuable insights on the Knowledge, attitude, potential factors, including sociodemographic aspects and the current trends/practices of weaning among caregivers in Bahrain. The cross-sectional study was carried out at fourteen randomly selected local Mother and Child Health Clinics (CHCs) in the Kingdom of Bahrain. A total of 253 caregivers capable of communicating in Arabic and English and with infants between 6-24 months were included. Detailed demographic data was collected, and all study subjects were instructed to fill the knowledge, attitude and practice assessment questionnaire. Responses were analyzed using software R v4.1.0 and Microsoft Excel. A Chi-square test was used to check the association of demographic variables with knowledge, attitude and practice. P-value ≤ 0.05 was considered statistically significant. Although a significant number of the subjects had an excellent positive attitude (67.59%), a high number of the subjects had inadequate knowledge (58.89%) and followed inappropriate practices (54.94%) concerning the introduction of supplementary food. The mother's education level and order of the child were significantly associated with knowledge, attitude, and practice. More prenatal and postnatal health promotion activities are required to shed light on the benefits of the timely and appropriate introduction of supplementary food..

Abstract : Spinal anaesthesia is the most popular regional anaesthesia technique for lower limb orthopaedic and lower abdominal surgery. Localanaesthetic agents as sole doesn’t have the advantage of postoperative prolonged analgesia. Nalbuphine being an opioid agonist antagonist was studied as adjunct to intrathecal Bupivacaine in different doses by different authors. We have decided to study intrathecal Nalbuphine 0.5 mg added to Bupivacaine against plain Bupivacaine. We conducted study to know effectiveness of intrathecal Nalbuphine on characteristics of subarachnoid block such as sensory block, motor block and postoperative analgesia. After randomization Group A patient received intrathecal Bupivacaine 0.5% heavy whereas Group B patients received intrathecal Bupivacaine 0.5% heavy with Nalbuphine 0.5 mg. Sensory and motor onset characteristics are significantly faster in Group B as compared to Group A. Duration of analgesia was significantly prolonged in Group B as compared to Group A with significantly less number of post operative analgesic requirement. Side effects are minimal and easily treatable in both groups. We recommend that 0.5 mg Nalbuphine as an adjunct to 0.5% Bupivacaine significantly produce favorable sensory and motor characteristics with good post operative analgesia with negligible side effects..
