Abstract :

 25 year old male, working as a mechanic presented to the ophthalmology OPD with complaints of painless diminution of vision since 15 days. He had no other ocular symptoms and was a known case of schizophrenia maintaining well on psychotropic medication. His family history was not significant and no high risk behaviour was seen. Ocular examination showed a vision of 6/60 (OD) and Hand Movements (OS) with features of Neuroretinitis in OU fundus. His routine lab investigations were within normal limits, with a negative VDRL but 1:160 titres on TPHA in serum and 1:80 in CSF. Patient was diagnosed as a case of neurosyphilis and treated for the same with remarkable improvement in ocular symptoms with vision 6/9 (OD) and 6/36 (OS) after 2 weeks. Due to the high risk and an aggressive presentation of behavioural symptoms the department of psychiatry decided against discontinuing there medications. Patient reports to be doing well and is on regular follow up.

Neurosyphilis is a tertiary manifestation of Syphilis which can present as neurological, behavioural and ocular  signs. Several studies  have reported a shift in the trend of clinical manifestations of the disease in the recent past. Psychiatric symptoms may range from mood features, dementia or delirium and psychosis. Ocular syphilis with an incidence of 0.65% can involve any part of the eye leading to an inflammatory response. Of these neuroretinitis is a rare complication with risk of loss of vision. As psychiatric symptoms can be the presenting manifestation in patients of syphilis, intervention can be delayed leading to further systemic involvement.

Abstract : Most clavicles have been treated properly, but currently, there is an increasing trend towards the effective management of clavicle fractures. Aim is to compare the radiological and functional effects of conservative treatment and operative management in the patients with clavicle midshaft fracture. 30 patients with fracture midshaft of the clavicle were randomly assigned to operative or nonoperative treatment. Results had been analysed based on the results of the operation with the radiological union based on the Constant- Murley score. There were no differences between the two groups in terms of the number of patients, the pattern of injury, or the pattern of fracture between patients who completed the six-month follow-up. Between the two groups of subjects, the Constant-Murley score was similar. Compared with the non-operative group, the operative group union time was 14.57 weeks compared with 16.04 weeks of a non-operative group. Two non-union cases were detected in the non-operative group and one in the operative group. After six months, the patients in the operative group were satisfied with the results regarding their condition compared to those in the non-operative group. An improved clinical and functional outcome is not achieved by plating displaced clavicular fractures after six months of follow-up, but the likelihood of non-union is significantly reduced. Nevertheless, patients in the operative group were more likely to be satisfied than their counterparts in the non-operative group. In light of this, we suggest a patient-specific, individualized treatment, which takes into account functional demands, fracture characteristics, and non-union risk factors..

Abstract : Acute appendicitis is one of the common surgical emergencies. This study analysed C- reactive protein as a diagnostic marker of acute appendicitis. To study the efficacy of C-reactive protein in the diagnosis of acute appendicitis. A prospective study done on patients coming to hospital with clinical diagnosis of acute appendicitis and underwent appendectomy. Operative and histopathological findings were compared and analyzed with C- reactive protein. A total of 100 patients were included in the study. CRP was more than 6mg/l in 89 patients, and out of 89 patients, 87 patients had histopathologically inflamed appendix and serum CRP estimation in diagnosis of acute appendicitis yield sensitivity of 90.6%, specificity of 50%, positive predictive value of 97.75% and negative predictive value of 18.2%. CRP was raised in patients with acute appendicitis and it aids as a marker for diagnosing acute appendicitis and decreasing the rate of negative appendectomy..

Abstract : Hemodialysis (HD) is the major modality of renal replacement therapy in 70% to 90% of patients. Vascular access is a crucial requirement when performing hemodialysis. The three most common types of vascular access used in clinical practice are arteriovenous fistula (AVF), arteriovenous graft (AVG), and central venous catheters (CVC). In this study, we aimed to understand the association of age of patients with a type of vascular access chosen by the vascular surgeon in Gorgan, Iran. In this cross-sectional study, all the new Hemodialysis (HD) patients (Incidence) and prevalent HD patients underwent hemodialysis in 2 academic hospitals (5 Azar and Sayyad Shirazi) from March 2020 to March 2021 were enrolled. In this study, the association between the age of ESRD patients and the type of vascular access was evaluated. According to One Way ANOVA analysis, there was a statistical difference in the mean age of patients in each starting access group of patients (P-value: 0.00). In PostHoc analysis, there was a statistical difference between the mean age of the AVF group, as starting vascular access, and permanent /Temporary catheters respectively (P:0.017& 0.00). Each individual’s decision of vascular access remains challenging. Placement of an AVF therefore may not be justified if the patient’s life expectancy is predicted to be too short to reap the long-term benefits of a native AVF..

Abstract : Ectopic tooth outside the oral cavity is infrequently reported in the literature. They may present in a variety of ways such as chronic or recurrent sinusitis, sepsis, nasolacrimal duct obstruction, headaches, symptoms of osteomeatal complex block and facial numbness [1]. They may present with symptoms based on their abnormal positions or at times may be asymptomatic however once diagnosed with a view to avert any impending complications, most ectopic teeth are surgically removed. Ectopic teeth which are asymptomatic and are found incidentally during routine examinations, may be followed up periodically and observed. Once clinical examinations show evidence of an abnormality, the diagnosis is confirmed with help of imaging studies such as CT-PNS and orthopantomograms. CT being the gold standard diagnostic test [2]. The approaches to removing such foreign bodies, was classically the Caldwell Luc approach, however this is being widely replaced by the endoscopic approach due to its lower morbidity rates and patient preference.
